Welcome. Completing your questionnaires is the first major step in your immigration case process with Sumner Immigration Law. The information you provide allows us to draft the necessary forms accurately and efficiently.

Please read the following information carefully before getting started.

Before You Begin: Special Circumstances

Please contact our office immediately before completing your questionnaires if any of the following apply to you:

These situations may affect your eligibility or the legal strategy for your case, and it is important that we are aware of them upfront:

  • You have a criminal record
  • You have ever been a member of the Communist Party
  • You have a history of drug or alcohol use
  • You have ever had a U.S. immigration violation (including overstays, prior removals, unlawful entries, or visa fraud)

If any of the above apply, please contact us before proceeding so we can advise you accordingly.


General Instructions

  • The questionnaires are case sensitive. Use proper spelling, capitalization, and punctuation throughout.
  • Provide accurate and complete information. If you are unsure of a response, leave it blank—we will follow up with you.
  • Do not click “submit and inform” until you have completed all assigned questionnaires. Doing so before completing all forms will lock you out of the system.

How to View and Complete the Questionnaire in Your Native Language

  • When you receive our questionnaire, click the Access link provided in the email.
  • Enter the password also included in the email to open the questionnaire.
  • On that page, click on the questionnaire you need to complete.
  • At the top of the screen, you will see a Google Translate toolbar.
  • Use the drop-down menu in that toolbar to select your native language. This will translate the questions into your selected language.
  • You may also type your responses in your native language.

Name Consistency

A note for foreign national spouses: If you plan to take your partner’s last name, please enter your name accordingly in the questionnaires. The name listed will appear on your green card, Social Security card, and driver’s license. These documents should reflect a consistent name for any future applications.

Information You Will Likely Need

Each questionnaire is customized to your case type. However, the following categories of information are commonly required. Please read the questionnaire(s) assigned to you carefully and gather as much information as possible before starting.

Common information includes:
  • 5-year residence history (or longer in some cases)
  • 5–10 year employment/schooling/unemployment history, including your last address and last job/school abroad if applicable
  • Full information for each parent (name, date/place of birth, current residence or year of death)
  • Information on all current and former spouses
  • Information on all biological, adopted, or current stepchildren—whether or not they are applying with you
  • Dates and details of any travel outside the U.S. in the past five years
  • Military service history, if applicable
  • Languages you speak and/or read
  • Biographic details (e.g., height, weight, eye and hair color)
  • USCIS A# or other immigration ID numbers, if applicable
  • Location of any prior immigrant visa issuance or adjustment of status
If financial sponsorship is required, you will also need:
  • Adjusted Gross Income (AGI) from the past three federal tax returns (as shown on your tax returns, not W-2s)
  • Documentation of assets, if needed to supplement your income

If your income does not meet 125% of the federal poverty guidelines for your household size (100% for active-duty military), please contact our office immediately so we can review available options, such as joint sponsorship or use of qualifying assets.

Final Reminders

  • Please follow each questionnaire’s instructions carefully. Your thoroughness at this stage helps prevent delays later.
  • Some forms request information older than five years (such as your last foreign address or job). Please include that information, even if it is outside the typical timeframe.
  • Your questionnaires are for internal use only. They are not submitted directly to USCIS. We will use your responses to prepare your immigration forms.
